How the Federal Program Is Built
Original Medicare is the federal program itself, not a private plan. It has two pieces. Part A covers care when you are admitted, and Part B covers doctors, tests and outpatient services.
Knowing how those two pieces work is the foundation for every other choice, whether you later add a supplement, pick a drug plan or move to Medicare Advantage.

We do not offer every plan available in your area. Any information we provide is limited to those plans we do offer in your area. Please contact Medicare.gov or 1-800-MEDICARE to get information on all of your options.
Part A: Hospital Coverage
Part A pays for inpatient hospital stays, a limited period of skilled nursing care after a qualifying admission, hospice and some home health care. If you or your spouse paid Medicare payroll taxes for about ten years, you owe no premium for it.
You owe $1,736 in 2026 at the start of each benefit period, which opens when you are admitted and closes once you go two straight months without hospital or skilled nursing care, so a rough year can include more than one.
Part B: Doctors and Outpatient Care
Part B covers physician visits, lab work, imaging, preventive screenings, durable medical equipment and outpatient surgery. The standard premium this year is $202.90 a month, typically deducted from Social Security, and the annual deductible is $283.
Once that is met, your usual share is one fifth of Medicare's approved charge. People with high incomes also pay an extra amount known as IRMAA.
What Original Medicare Does Not Pay
- Routine dental work and dentures
- Routine eye exams and most glasses
- Hearing aids and fittings
- Most outpatient prescriptions, which need Part D
- Long-term custodial care
- Any annual limit on your share of costs
Those gaps are why most people add something. Our Medigap guide and Medicare Advantage guide explain the two main paths.
When Not to Delay Part B
If you do not have job-based coverage from current employment, delaying Part B can result in a lifetime penalty. Active coverage through a large employer, your own or a spouse's, may let you wait. Ask before you decline it, and see our turning 65 page for the timeline.
A Free Second Opinion on Your Coverage
