Matching Drug Coverage to Your Pill Organizer

Two neighbors on the same block can pay wildly different amounts for the same drug plan. The reason is simple: each plan has its own formulary, pricing tiers and preferred pharmacies, and those details only matter once they meet your specific prescriptions.

You can get drug benefits through a separate Part D policy paired with Original Medicare, or inside a Medicare Advantage plan that includes them. Either way, the right choice starts with an accurate list of your medicines and doses.

We enter that list into the plans we carry and show you a full-year estimate, not just the monthly premium.

What Changed for 2026

Your yearly share of covered prescription costs now stops at $2,100. Reach that point and the plan pays all covered drugs through the end of December. Formulary insulin costs no more than $35 a month, and CDC-recommended adult vaccines are free under Part D.

When January refills hit hard, an optional monthly payment program from Medicare lets you spread what you owe into monthly payments instead of paying everything at the counter.

Reading a Formulary

  • Tiers set your price, from low-cost generics up to specialty drugs
  • Prior authorization means the plan must approve a drug before paying
  • Step therapy asks you to try a cheaper drug first
  • Quantity limits cap how much you can fill at one time

If a drug you need is missing or restricted, your prescriber can ask the plan for an exception. We point out those hurdles before you enroll so they do not surprise you at the pharmacy.

Pharmacy Choice Matters

Copays often drop at a plan's preferred stores or by mail. We check the store you already use, whether it is a chain on Route 1 or a neighborhood shop on South Broad Street, and show how prices change if you switch.

The Late Enrollment Penalty

If you go without Part D or other creditable coverage for a stretch of 63 days or longer after you first qualify, Medicare adds a penalty to your drug premium. It grows with each month you were uncovered and usually stays for as long as you have Part D.

Coverage through a large employer or union often counts as creditable. Your plan should mail a notice each year confirming whether it does; keep it with your records.

Help With Drug Costs

Extra Help, also called the Low-Income Subsidy, can lower premiums and copays sharply for people with limited income. New Jersey residents may also qualify for PAAD or Senior Gold. Our savings programs page explains who qualifies.

A Free Second Opinion on Your Coverage

Jot down your prescriptions and the names of the doctors you see. We line up what you have now against the plans we represent and point out the real differences in cost and in where you can get care. Expect about thirty minutes, with no decision required during the call. You can also read our background before calling.

What Trenton Neighbors Ask About Part D

I take no medicines. Should I still get Part D?

Usually yes. A low-cost plan now avoids a permanent penalty later, unless you have other creditable drug coverage.

Can I change drug plans during the year?

Typically only during fall Open Enrollment, though moving, getting Extra Help and some other events allow changes at other times.

What if my plan stops covering a drug?

Ask your prescriber about an exception or a similar drug on the list, and review options during the next enrollment season.

Does Medicare Advantage include drug coverage?

Most Part C plans do. In that case a stand-alone drug policy is unnecessary.
